This affidavit assures the Family League that they will contract with a vendor that is legally recognized by the State of Maryland. Furthermore, it ensures that the applicant has reviewed and is accepting of the Standard Terms and Conditions.

Applicants are required to submit with their application a letter of commitment outlining a basic partnership agreement that is signed by both the applicant and the principal from their partner school. This sample letter is provided as guidance for this attachment.
