This is a Microsoft Excel file. The Family League of Baltimore City's Annual Budget form; to be filled out and submitted with application (does not count towards narrative page limit). Please be sure that the Budget Form matches costs outlined in the Budget Narrative and that the required matching costs are reflected in the Other Funding column.


Baltimore City YouthWorks Executive Order & Form: Baltimore City YouthWorks Form.pdf
This Executive Order encourages local businesses and contractors, service providers, vendors, etc. to support the City’s efforts to provide employment opportunities for Baltimore’s youth. The Baltimore City Youthworks Form must be completed and returned with the proposal.


Baltimore Residents First (BRF) Executive Order, Certification Form & Employment Report: Baltimore City Residents First Certification Statement.pdf
This Executive order creates opportunities for businesses that receive municipal contracts to access qualified City job seekers to meet their workforce needs. The BRF Certification Form must be completed and returned with the proposal. All vendors that are selected for funding must also complete the Employment Report as instructed throughout the term of the contract.

New Vendor Form
This form enables the Family League fiscal department to establish a payment mechanism for a new vendor.

Proposal Affidavit: Affidavit 2009.pdf
This affidavit assures the Family League that they will contract with a vendor that is legally recognized by the State of Maryland. Furthermore, it ensures that the applicant has reviewed and is accepting of the Standard Terms and Conditions.


Proposal Cover Page: Proposal Cover Page After School.pdf
The proposal cover page is to be fully completed by the applicant and should serve as the top page of the completed proposal.

Standard Terms and Conditions: Standard Terms and Conditions FY 2009.pdf
All applicants must read these terms and conditions prior to submitting a proposal. These standard terms and conditions are a part of every contract issued by the Family League.


W-9 Form: W-9 Form.pdf
This form enables the Family League to obtain an applicant’s taxpayer identification number so that we may properly report information to the IRS.
