The Criminal Background Checks letter template can be used by the Executive Director of a program ensuring the contract management team of FLBC that the staff and volunteers listed in the letter have up-to-date, legitimate background checks without any incriminating information which would prevent them from being able to work with children.

It is important to note that the attached consent form is now valid for three (3) years. You will no longer input consent data each year for returning youth. New consent data should be entered for new students/youth attending your program.

The Expenditure Form is a financial form in which the fiscal person(s) from every program submits to The Family League of Baltimore City, Inc. every quarter informing The Family League of how they spent their money from the previous quarter.

The Reporting Chart is an inclusive list of the dates that documents are needed from every after-school provider to FLBC.

In an effort to build capacity, ensure accountability, evaluate results and link funding to performance, programs funded through Baltimore’s After School Strategy are required to comply with the standards as a condition of their grant contract.
